The problem

My main research interests are pathophysiology and treatment of complications of cirrhosis: ascites, acute kidney injury (AKI), hepatorenal syndrome (HRS), bacterial infections, and acute-on-chronic liver failure (ACLF).

The approach

I carried out several clinical studies and randomized controlled clinical trials on complications of cirrhosis, which contributed to change the management of patients with chronic liver disease.

Main discoveries and innovations

  • Definition of new diagnostic criteria for AKI and HRS
  • Development of an algorithm for the management of AKI which is currently recommended by EASL clinical practice guidelines
  • Demonstration of the safest strategy to use terlipressin in patients with HRS, how to predict response to terlipressin and albumin in patients with HRS and what is the impact of response to treatment on clinical outcomes of liver transplant candidates/recipients
  • Development of a new protocol of antibiotic treatment for nosocomial spontaneous bacterial peritonitis
  • Clarification of the epidemiology of bacterial infections in cirrhosis worldwide and risk factors for multidrug resistant bacteria
  • Redefinition of sepsis in cirrhosis 
  • Contribution to  understanding the pathophysiology, risk factors, clinical course and outcomes of ACLF
